Assistance Intake Process

STEP 1  

Meet with HOPE staff for intake session and to determine eligibility.  You may submit a request for assistance by visiting the HOPE office located in the HOPE Ministry Center, 680 W. Main Street, Wytheville, or by calling the HOPE intake desk at 228-6280, Ext. 210, or toll free at 877-818-8680, Ext. 210.

REQUESTS FOR ASSISTANCE WILL ONLY BE ACCEPTED DURING THE FIRST FULL WEEK AND THE THIRD FULL WEEK OF EACH MONTH DURING THE FOLLOWING HOURS:

Monday through Thursday, 9:00 a.m. to 3:00 p.m.
Friday, 9:00 a.m. to 12:00 noon

STEP 2  

If it is determined that you meet the minimum eligibility requirements for one or more of HOPE’s assistance programs, be advised that:

bulletEligibility does not guarantee that you will receive financial assistance from HOPE.
 
bulletDue to very limited funding, HOPE will use a random drawing (“lottery”) to determine which eligible families/individuals will receive financial assistance.
 
bulletIf you are selected to receive financial assistance from one of HOPE’s programs, you will be contacted via telephone between the hours of 12:00 noon and 4:00 p.m. on Friday of the week you applied for assistance.  At that time, an appointment will be scheduled for you to meet with a housing counselor to complete necessary paperwork.
 
bulletIf you miss your appointment, you will forfeit financial assistance and you will have to reapply at a later date.
 
bulletYou will be required to verify the information you have reported to us before financial assistance is provided. If you are unable to present verifying documentation on the date of your appointment, you will be determined ineligible and will have to reapply at a later date.
 
bulletA checklist of required documentation is provided below.  Please gather this documentation ahead of time so it will be ready should you be selected for financial assistance.
 
bulletEligible clients who are not selected to receive assistance will be notified by mail and may reapply during the next intake cycle.

STEP 3  

Be prompt for your appointment with the Housing Counselor and bring the following required documentation:

Driver’s license or state issued photo ID for all household members over 18 years old.

Social Security card for all household members.

Verification of employment for all household members currently employed.

¨

Verification of all household benefits you receive:

    ¨ Food stamps

    ¨ TANF

    ¨ Child support
    
¨ Unemployment benefits

    ¨ Social Security retirement
    
¨ SSI

    ¨ Social Security disability
    
¨ VA retirement/disability benefits

    ¨ Short-term/long-term disability insurance benefits
   
¨ Other benefits documentation as determined

¨

Pay or quit notice from landlord or court summons for eviction.

¨

Statement from landlord of deposit and first month's rent amounts needed.

¨

Signed Lease/Rental Agreement.

¨

Most recent statement from your mortgage company.

¨

Most recent utility bill termination notice.

¨

Written verification of utility deposit amounts needed.

¨

Proof of job layoff or reduced hours at work - written statement from employer or from VEC office.

¨

Proof of illness/injury which caused the primary wage earner to miss work (doctor’s excuse, statement from employer, etc.)

¨

Proof of exhaustion of unemployment benefits – written statement from VEC.

¨

Proof that you are in the process of applying for disability benefits – written statement from Social Security.

¨

Proof of homelessness – written verification from shelter.

¨

Other documentation as determined.

It is important that you comply with all requests of the Housing Counselor. Additional forms, documentation or actions may be required.
